Discontinuation of Membership in the Wellcome Program

A. Discontinuation of Membership Not Initiated by the Member

Maccabi may discontinue a member's membership in the program for any of the following reasons:
1. Delays in payment of membership fees - Maccabi may end the membership of a member of the program and negate their eligibility for medical services if the member does not pay membership fees. The member will not be entitled to receive medical services by virtue of the program beginning in the first month for which they have failed to pay membership fees. The member will be required to reimburse Maccabi with the cost of the medical services they were provided, if they were provided, in each month for which the fees were not paid by the member.
If the member proves that membership fee payments were halted unbeknownst to them and/or that they did not initiate it, the member may request to renew their membership in the program if no more than 30 days have passed since they were informed of the discontinuation of their membership and they have fully paid their debts, in cash.

2. False declarations and false information - Membership may be discontinued at any time if it is revealed that the member's declarations, including but not limited to the health declaration in the program membership application, are untrue and/or incomplete.
The acceptance of a member to the program is based on the declarations they provided in writing to Maccabi. The veracity and completeness of the information provided is essential. If the member has provided incomplete and/or dishonest answers to the questions in the application form or they have failed to bring to Maccabi's attention facts that would have influenced their being accepted or rejected for the program, Maccabi may discontinue their membership in the program immediately.
Maccabi may charge the member for any expenses incurred if the member's declarations, including the health declaration, are not true and complete.

3. Maccabi has learned that the member is staying in Israel illegally.

4. The member conducted themselves in an inappropriate manner, physically and/or verbally, towards Maccabi and/or its employees and/or its physicians and/or its members.

B. Discontinuation of Membership Initiated by the Member

A member may discontinue their member by filling out and signing the Request for Discontinuation of Membership form. The discontinuation of membership will apply from the day the request is delivered to a Maccabi branch, subject to the program's terms, and in any case those who want to end their membership must meet the mandatory payment of 4 months' fees and settle any debts they may still have.
Discontinuation of membership will also discontinue the nursing care insurance, and for those who are not Maccabi members, supplementary insurance will also be discontinued.
As stated above, membership in the program will be valid until discontinued at the member's request, voluntarily and in writing as specified in this section or by Maccabi as stated in section A above.
Ceasing to pay membership fees for the program will not be considered as a notification of discontinuation and the member's debt will still require settlement, even after their membership has been discontinued.
A member who has paid membership fees for several months in advance and wants to discontinue their membership can be reimbursed of the membership fees they have paid for the period after the discontinuation of membership, except for the fees for the first 4 months which were paid at the time of acceptance to the program.
No reimbursement will be made for parts of a month, and the reimbursement will be made starting from the month following the one in which the membership was discontinued.

C. Cancelling an Application

The application to join the program can be cancelled and the paid membership fees reimbursed in full, provided the cancellation takes place in writing within 2 business days from the time of acceptance to the program and if no use of medical services was made through the program.
A full reimbursement for fees paid will be transferred to the member's bank account. If more than 2 business days have passed, this will be considered a discontinuation of membership initiated by the member, subject to section B above.
