The Welcome program entitles its members to join the supplementary programs. Members of the program may join supplementary insurance programs Maccabi Sheli and Maccabi Zahav, as well as nursing care insurance Maccabi Siudi and the Maccabi Fund (Keren Maccabi).

The Maccabi Fund (Keren Maccabi) membership fees are 0.71 NIS per month for members over 19 years old, last updated on July 2016.
Membership Fees Discount While Staying Abroad
The Wellcome program offers its members an exclusive benefit: members of the program staying outside of Israel for at least 3 consecutive months are entitled to a 75% discount on membership fees during their stay abroad, subject to the following terms, while maintaining continuity of entitlements - with no time limit.
Terms and Clarifications for the Overseas Discount
- The discount applies only to membership fees in the basic program and does not apply to fees for supplementary insurance, nursing care insurance, or the Maccabi Fund.
- The discount is provided on the condition that the member does not use any services, including purchase of drugs or any other service, at Maccabi's expense.
- The discount will be valid from the month following the month of the member's departure and until the month before the month of the member's return. For example, a member who left in March and returned in September will be entitled to the overseas discount for the months of April through August.
- The discount only applies to those staying abroad and does not apply to those visiting/staying in Israel, even if they did not use any medical services.
- -The overseas discount will only be applied retroactively, when presenting the passport entry and exit listings at a Maccabi branch. If the passport control was performed in the biometric method or the member does not present their passport at a Maccabi branch, they must present an up-to-date summary from the Ministry of Interior including arrivals in and departures from Israel.
- A retroactive reimbursement may be requested for such a discount that has not yet been given, subject to the following conditions:
A. The discount is for a period no longer than 12 consecutive months.
B. The request is submitted in writing.
C. The request is submitted within 3 months of the end of the period for which the discount is being requested.
D. The Member's passport is presented at a Maccabi branch, listing arrivals in and departures from Israel. As specified above, if the passport control was performed in the biometric method or the member would like to request the discount without visiting a Maccabi branch, they must present an up-to-date summary from the Ministry of Interior including arrivals in and departures from Israel.
E. Provided the member was eligible for the discount during the specified period as stated above.
F. Provided the member did not use any medical services in the period for which they request the overseas discount. - Those residing in Israel are not entitled to a discount at any time, even when services are not used. Non-use of medical services does not exempt a member from paying fees at the full rates and does not qualify a member for the overseas discount.
No membership fees will be reimbursed for months in which medical services were not used. - A member who paid the discounted rate (overseas discount) and did not meet the stipulations to qualify for the discount may be charged the difference by Maccabi. The charge will be made using the payment method through which membership fees for the program are charged.
Family Discount Rate
Members of the Welcome program can benefit from a reduced family rate:
20% discount when one adult and at least two minors are members of the program,
30% discount when two adults and at least one minor are members of the program.
Clarifications for the Family Discount Rate
- New members who are entitled to the reduced family rate will be given the discount immediately when being accepted in the program.
- Any change in family's makeup (a family member joining/leaving, dissolution, unification) will result in updated rates starting from the month following the change.
- Discounts cannot be combined. Members entitled to multiple discounts (overseas discount, family discount, etc.) will pay the lowest rate they are entitled to.
- The reduced rate applies only to the program membership fees and not the fees for supplementary insurance, nursing care insurance, or Maccabi Fund
- The discount is not given retroactively.
- For the sake of clarity, a family for purposes of the Family Discount Rate is a nuclear family: parents and their minor children.
Monthly payments
- Maccabi may determine an addition to the regular rates, according to the medical condition of a member joining the program.
- Payment of membership fees will take place through a direct debit authorization for an Israeli bank account or through a credit card, in New Israeli Shekels only.
- When joining the program and as a precondition for completing the process, members will pay membership fees for the first 4 months, regardless of the length of membership they want. This fee will be collected in advance one time only. Collection for the first month will be in accordance with the date of acceptance in the program. Maccabi may require an advance payment for a longer period, at its discretion.
- Afterwards, the payment will take place through a direct debit authorization that takes place regularly, according to Maccabi regulations.
- Members of the program who have paid in advance for the minimum period and whose status was changed by the National Insurance Institute to residents entitled to medical services by law, will be entitled to a shorter membership period and a reimbursement of the difference for the period after their eligibility was restored.
- As a rule, no partial months shall be reimbursed, except for those who have finished their waiting period and became residents legally entitled to medical services.
- Members of the program are obligated to pay any and all deductibles for services/treatments that Maccabi may charge under the National Health Insurance Law and/or the collection plans approved for the health service provider, and are not entitled to benefits and/or caps and/or exemptions to these payments that are granted to Maccabi members.
- The payment of fees for the supplementary insurances, nursing care insurance, and Maccabi Fund will be collected in addition to program membership fees.

Who may join?
Anyone residing in Israel in a legal manner whom the Israeli National Health Insurance Law doesn't apply to
can join, such as:
- Tourists, diplomats, foreign citizens, foreign
- exchange students.
- People who prefer not to gain Resident status
- with the National Insurance Institute.
- Family members of Israelis who have not yet
- gained Resident status.
- Children brought into Israel for adoption, who
- have not yet been granted Israeli citizenship.
- Israeli residents in a waiting period under section
- 58 of the Israeli National Health Insurance Law.

What is section 58 of the Israeli National Health Insurance Law?
The Wellcome program allows residents in a waiting period under section 58 to benefit from all of Maccabi's health services.
Under section 58 of the National Health Insurance Law, an Israeli resident returning after 2 years or longer abroad, after March 1, 2001, who has not paid National Health Insurance dues, or is behind on payments for over 12 months, loses the right to health services in Israel.
Those who have lived abroad, ceased to be Israeli residents, and later move to Israel again and have been recognized as residents by the National Insurance Institute, are also not automatically entitled by law to receiving health services.
To become eligible again for health services, there is a one month waiting period for every year of absence from Israel for those who left after November 1, 2008. For those who left earlier, the waiting period is 2 months for each year of absence. A year of absence from Israel is a year during which the resident lived outside of Israel for at least 182 days, even if they were not consecutive.
In any case, the waiting period will be at least 2 months and no more than 6 months. The waiting period can be redeemed with a special payment to the National Insurance Institute of Israel. Immediately after this special payment medical services can be used.
Eligibility for a waiting period is determined by the National Insurance Institute and the information is then sent to the health service providers.

- Maccabi may update and/or change the details of the program from time to time, including the rights of
program members. Said updates and/or changes will be effective immediately when published and will apply to all program members at that time, unless stated otherwise. - All terms of the program will apply equally to all program members regardless of the health service provider
they were members of before joining the Welcome program, unless stated otherwise in the program
regulations. - In addition to the Wellcome program regulations, members of the program are also subject to the Maccabi
regulations and the health service provider rules.

Services included in the Wellcome program
The services provided by the Wellcome program match those included in the health services provided by the National Health Insurance Law, except for the exceptions below, and in accordance with the program's rules and regulations. The medical services are provided within Israel only.
In any case, members of the program will not be eligible for services beyond the package of services specified in the legislation.
Which Services are Included?
- Primary care: General practitioner, pediatrics, and obstetrics and gynecology.
- Specialized care.
- Free dental care up to the age of 14.
- Emergency medicine, hospitalization, and emergency room care.
- Elective hospital admission.
- Care at children's health clinics.
- Diagnostic services at Maccabi facilities, including imaging and gastroenterology.
- Laboratory tests.
- Medical drugs covered by the National Health Insurance Law.
- Treatment in the Dead Sea for Psoriasis patients.
- Child development treatments.
Program Restrictions
Members of the Wellcome program will not be entitled to these services:
1. Hospital admission during the first 3 months of membership in the program, other than acute (emergency) hospitalization.
2. Treatment of pre-existing conditions, including deterioration.
3. Treatment of severe and chronic illnesses, as defined by the Israeli Ministry of Health, if they are diagnosed during the first 6 months of program membership.
4. Treatments or tests related to pregnancy and childbirth, including but not limited to: childbirth expenses, treatment of a newborn (including premature babies), hospitalization due to pregnancy complications, miscarriage, abortion, etc.
5. Fertility treatments.
6. Treatments directly and/or indirectly related to and/or caused by a car accident in Israel or abroad.
7. Treatments directly and/or indirectly related to and/or caused by a workplace accident.
8. Treatment and hospitalization for impotence and sexual dysfunction.
9. Treatment in relation to mental health, intentional self-harm, or attempted suicide.
10. Treatment for injury caused in scuba diving, skydiving, hunting, ski, and any other extreme sports.
11. Treatment or injury caused in acts of hostility, war, enmity, terrorism, or crime.
12. Treatment of illnesses caused by the consumption of alcohol or illegal drugs.
The restriction in clause 1 above does not apply to those who have retained continuous privileges from when they were residents, meaning they joined the Welcome program within one month of when they were informed in writing that they are no longer residents, if they registered for the Welcome program before the hospitalization was needed or before the severe or chronic disease was diagnosed.
Maccabi may update and/or change the details of the program from time to time, including the rights of the program members.

How to Join
To join Maccabi's Welcome program (formerly the passport-holders' program, or Darkonaim) you must fill out an application form, including a health declaration. This form will then be delivered, along with a photocopy of your Israeli ID or your passport and relevant medical documents to a Maccabi branch.
The health declaration is provided in Russian, French, and Arabic for assistance purposes only. The application form, including the health declaration, must be filled out in either English or Hebrew!

When joining the program and as a condition for the completion of the process, members will pay membership fees for 4 months, regardless of the length of membership they want. This fee will be collected in advance one time only. Payment of membership fees and other dues (drug costs, doctor fees, etc.) will be conducted through a direct debit authorization for an Israeli bank account or through a credit card, in New Israeli Shekels only.
Applicants staying abroad must fill out the application form, sign where so instructed and mail the form to a Maccabi branch, attaching a photocopy of their Israeli ID or their passport and a direct debit authorization signed by the bank. If necessary, enclose relevant medical files as well.
Applicants who receive a letter notifying them of the cessation of eligibility for medical services by law will fill out the registration form included with the letter and send it to a Maccabi branch, attaching medical documents as necessary.

Clarifications
Clarifications for those joining the Welcome program
1. Approval to join the Wellcome program will be given at Maccabi Health Services' sole discretion and subject to the program terms and regulations as specified in this document.
2. Any request to join will be individually considered and approved subject to the limitations stated in the program.
3. Any request to join the program, including by a Maccabi member by virtue of the National Health Insurance law (1994) whose residency has been rescinded, is subject to prior approval.
4. Maccabi holds authority to accept or reject an application to join the Welcome program without having to justify its decision.
5. Wellcome program applicants must, at Maccabi's request, make themselves available for medical examination, at their own expense, and submit any detail and/or medical document that is requested within reason. This clause is a pre-condition to consideration of the application to join the program.
6. Maccabi may approve a member's application while imposing limitations on certain treatments, subject to their medical state.
7. The relationship within the Wellcome program is solely between Maccabi and the member who is signatory to the program application form.
8. Members will only be entitled to receive medical services through the program after resolving payment of membership fees and receiving Maccabi's confirmation of acceptance to the program.
9. Maccabi is neither obligated nor responsible for the period between the application to join the program and the acceptance into the program.
10. Maccabi will be exempt from providing services related to birth defects and congenital defects, including but not limited to hereditary illnesses and/or medical conditions and/or medical phenomena and/or diseases whether treated or not and/or their results, whether directly or indirectly, which were caused and/or worsened due to a medical condition from before joining the program.
11. Residents of Israel in a waiting period under section 58 of the National Health Insurance law are obligated to inform Maccabi of the end of their waiting period.
12. The program application form and the health declaration are valid for one month from the day the declaration was signed. After this period those who wish to join are required to certify that there has been no change in their medical condition since their initial signature, provided no more than 3 months have passed since the original signature.
13. If a Wellcome member has used services for which they were not entitled as part of the program, or due to restrictions on receiving the treatment, Maccabi will be entitled to collect the costs of the treatment/service from the bank account or credit card from which membership fees are collected.
14. As a rule, a legal minor may be registered for the program only with a parent or legal guardian who is a Maccabi member.
In exceptional cases a minor may be registered to the program by a legal representative guardian who is a Maccabi member, subject to the minor's parents signing over power of attorney.
If the minor's parents reside abroad, both parents will sign the power of attorney in the presence of the Israeli consul in the country in which they reside. The original power of attorney (not a photocopy) must be sent to a Maccabi branch, signed by the parents and by the consul, certifying he has identified them and they have signed in his presence, along with photocopies of the parents' and minor's passports.
If the minor's parents reside in Israel, they will sign the power of attorney in a Maccabi branch in front of a service coordinator, and present a passport or ID card.

15. Adding any additional family member to the program, including a recently born baby, who was not mentioned in the submitted application form, requires submitting a new application form for this family member.
16. Maccabi may require that Welcome members come to a Maccabi branch to update and authenticate their information.

Supplementary Insurance
Joining the Supplementary Insurance Programs
Members of the Wellcome program may ask to join Maccabi's supplementary insurance programs Magen Zahav and Maccabi Sheli, subject to the programs' regulation and terms and/or restrictions as they apply to members of the Welcome program.
Joining the Maccabi Sheli Supplementary Insurance Program
Members of the Wellcome program may join Maccabi Sheli.
Maccabi members in a waiting period under section 58 enrolled in the Wellcome program are eligible for all Maccabi Sheli entitlements. All other Wellcome members enrolled in Maccabi Sheli are eligible for all supplementary insurance entitlements except for surgery abroad.
Joining the Maccabi Zahav Supplementary Insurance Program
Members of the Wellcome program may join Maccabi Zahav.
Maccabi members in a waiting period under section 58 enrolled in the Wellcome program are eligible for al
l Magen Zahav entitlements. All other Wellcome members enrolled in Magen Zahav are eligible for all supplementary insurance entitlements except for IVF treatments.
A Returning Resident who is a Maccabi member in a waiting period under section 58 of the National Health Insurance Law and has joined a supplementary insurance program, is entitled while residing in Israel to medical services according to the terms of the supplementary insurance, but will not be entitled to the basic health services granted by the National Health Insurance Law.
Maintaining Continuity of Entitlements in Supplementary Insurance
A member of the Wellcome program who was a member of a Maccabi supplementary insurance until such time as their entitlement to service by law was interrupted, can maintain their insurance continuity in the supplementary insurance program provided they joined the Wellcome program within 3 months of the interruption of their eligibility and have requested that said continuity be maintained in the process of joining. The continuity of entitlements will be maintained for the same level of insurance they were a member of before their eligibility was interrupted.
Maintaining continuity of entitlements is subject to the supplementary insurances terms as they apply to
members of the Wellcome program, and requires continuous payment of membership fee payments from the time when eligibility was interrupted.
Discontinuing membership in the program for any reason will cause discontinuation of membership in each of the insurance programs the member has joined.
It is hereby clarified that interruption of a Maccabi member's eligibility for medical services due to section 58 of the National Health Insurance law does not automatically discontinue the membership in supplementary insurances as long as the member continues paying membership fees for them, and does not require membership in the Welcome program for their membership in the supplementary insurances to continue.

Additional Programs
Joining Additional Programs
Joining Maccabi Siudi nursing care insurance
Any member of the Wellcome program may join Maccabi Siudi (Heb), subject to the rules and regulations of Clal Insurance Group's Health Division. Applicants need to fill out a separate application form for the nursing care insurance, in addition to the application form for the Welcome program.
Maintaining Continuity of Entitlements in Nursing Care Insurance
Members of the Wellcome program who were members of the Maccabi members' nursing care insurance until their legal eligibility for medical services was interrupted, can maintain continuity of entitlements for nursing care insurance if they have joined the program within a month of the interruption of entitlement and have asked to maintain said continuity in the process of joining the program.
Maintaining insurance continuity is subject to the insurance company's approval and maintaining a continuity in insurance fee payments since the eligibility was interrupted.
Joining the Maccabi Fund
Members of the Wellcome program are entitled to join the Maccabi Foundation (Keren Maccabi).

Discontinuation of Membership
Discontinuation of Membership in the Wellcome Program
A. Discontinuation of Membership Not Initiated by the Member
Maccabi may discontinue a member's membership in the program for any of the following reasons:
1. Delays in payment of membership fees - Maccabi may end the membership of a member of the program and negate their eligibility for medical services if the member does not pay membership fees. The member will not be entitled to receive medical services by virtue of the program beginning in the first month for which they have failed to pay membership fees. The member will be required to reimburse Maccabi with the cost of the medical services they were provided, if they were provided, in each month for which the fees were not paid by the member.
If the member proves that membership fee payments were halted unbeknownst to them and/or that they did not initiate it, the member may request to renew their membership in the program if no more than 30 days have passed since they were informed of the discontinuation of their membership and they have fully paid their debts, in cash.
2. False declarations and false information - Membership may be discontinued at any time if it is revealed that the member's declarations, including but not limited to the health declaration in the program membership application, are untrue and/or incomplete.
The acceptance of a member to the program is based on the declarations they provided in writing to Maccabi. The veracity and completeness of the information provided is essential. If the member has provided incomplete and/or dishonest answers to the questions in the application form or they have failed to bring to Maccabi's attention facts that would have influenced their being accepted or rejected for the program, Maccabi may discontinue their membership in the program immediately.
Maccabi may charge the member for any expenses incurred if the member's declarations, including the health declaration, are not true and complete.
3. Maccabi has learned that the member is staying in Israel illegally.
4. The member conducted themselves in an inappropriate manner, physically and/or verbally, towards Maccabi and/or its employees and/or its physicians and/or its members.
B. Discontinuation of Membership Initiated by the Member
A member may discontinue their member by filling out and signing the Request for Discontinuation of Membership form. The discontinuation of membership will apply from the day the request is delivered to a Maccabi branch, subject to the program's terms, and in any case those who want to end their membership must meet the mandatory payment of 4 months' fees and settle any debts they may still have.

Discontinuation of membership will also discontinue the nursing care insurance, and for those who are not Maccabi members, supplementary insurance will also be discontinued.
As stated above, membership in the program will be valid until discontinued at the member's request, voluntarily and in writing as specified in this section or by Maccabi as stated in section A above.
Ceasing to pay membership fees for the program will not be considered as a notification of discontinuation and the member's debt will still require settlement, even after their membership has been discontinued.
A member who has paid membership fees for several months in advance and wants to discontinue their membership can be reimbursed of the membership fees they have paid for the period after the discontinuation of membership, except for the fees for the first 4 months which were paid at the time of acceptance to the program.
No reimbursement will be made for parts of a month, and the reimbursement will be made starting from the month following the one in which the membership was discontinued.

C. Cancelling an Application
The application to join the program can be cancelled and the paid membership fees reimbursed in full, provided the cancellation takes place in writing within 2 business days from the time of acceptance to the program and if no use of medical services was made through the program.
A full reimbursement for fees paid will be transferred to the member's bank account. If more than 2 business days have passed, this will be considered a discontinuation of membership initiated by the member, subject to section B above.
